Incident reports

Incident reports give teachers and admins a formal way to write up something that happened — a behavior issue, a safety concern, anything worth a documented record — and send it straight to your co-op's board or a specific person. The feature is off by default.

What an incident report is

An incident report is a short write-up: a title, a description of what happened, and optionally the student and class it relates to. Once submitted, it can't be edited. That's intentional, so the record reflects exactly what was reported at the time. The only thing that can change afterward is its status: your board can mark a report resolved once it's been dealt with, or reopen it later if needed.

Turning it on

1Go to Admin → Settings and stay on the General tab.
2Find the Incident reports card and check Enable incident reports.
3Click Save.

Once enabled, teachers, admins, and owners each get an entry point to submit a write-up. You can turn it off again at any time. Existing reports aren't deleted, they're just no longer reachable until you re-enable it.

Writing up a report

Admins and owners open Incident Reports from the Admin Hub (under the Membership section). Teachers reach the same page from a link in the header of their Teaching page. It only appears once your co-op has turned the feature on.

Click New Report and fill in:

1Title — a short summary.
2What happened — the full description.
3Related student and related class (both optional) — link the report to a specific student or class if relevant.
4Send to — choose the whole board, or pick a specific admin to route it to directly.
5Attachments (optional) — attach photos or documents that support the report.
Note: Once you click Submit report, the title, description, and any attachments are locked in and can't be edited. Double-check it before sending.

Reviewing and resolving reports

Admins and owners (the "board") see every report submitted at their co-op. Teachers only see the reports they personally submitted. Click a report to expand it and read the full description and any attachments.

From the board view, each open report has a Mark resolved button. Resolved reports move into a collapsed Resolved section further down the page. Click to expand it if you need to review or reopen one later.
